核心内容提要 对于合并病态性肥胖的GERD患者外科治疗策略选择仍是临床诊疗工作的难点。特别是肥胖加剧GERD发生,而减重抗反流效果不一,当前全球开展最普遍的SG手术甚至可以增加GERD发生率,单纯的抗反流手术对肥胖病人又疗效有限,如何加强抗反流效果,合理选择减重术式是亟待考量的问题。对合并病态性肥胖的GERD患者,无论初始治疗还是单纯抗反流或减重术后的修正手术,RYGB都是目前较为理性的选择。近年,新型抗反流减重手术不断兴起,有望成为新的有效替代治疗措施。
