特异性药物性肝损伤(drug-induced liver injury, IDILI)是一种罕见但可能危及生命的事件, 其原因是药物在通常的治疗剂量下, 在大多数接受治疗的患者中不会引起任何肝损伤的生化或临床证据. IDILI最常见的临床表型是“急性肝炎”、“混合性肝细胞胆汁淤积性肝炎”和“胆汁淤积性肝炎”, 这些都是通过临床、生化和组织学特征来区分的. 抗微生物药、草药和膳食补充剂现在是药物性肝损伤中最常涉及的原因. 现已有几个量表被用来描述IDILI事件的严重性. 但目前尚无可靠的方法来准确预测IDILI事件的过程. 在临床试验中, “金标准”的肝脏安全信号是“海氏病例”的发生. 迄今为止, 药物性肝损伤在诊断(对乙酰氨基酚-半胱氨酸蛋白加合物)、预测(遗传生物标记物)和预后(microRNA-122、高迁移率族蛋白盒1、角蛋白-18、谷氨酸脱氢酶、线粒体DNA)的生物标记物的鉴定方面取得了进展. 其中许多生物标志物也提供了机制上的见解. 在治疗上近年无重大突破. |
