Cronkhite-Canada综合征(Cronkhite-Canada syndrome, CCS)由Wolsry Cronkhite Jr和Wilma Jeanne Canada于1955年首次报道, 1966年Jarnum和Jensen将本征命名为Cronkhite-Canada综合征, 是一种罕见的病因不明的非遗传性疾病. 其发病机制尚不明确, 暂无特异性治疗, 其治疗策略目前包括皮质类固醇激素、合成代谢类固醇、质子泵抑制剂(proton-pump inhibitor, PPI)、H2受体拮抗剂、营养支持、色甘酸钠、免疫抑制剂、抗生素、手术、5-氨基水杨酸、利妥昔单抗、根除幽门螺杆菌及这些疗法的组合, 近年来有使用粪便微生物群移植作为补充治疗对类固醇难治性CCS的报道, 但类固醇目前依然被认为是药物治疗的主要手段. 本病例采用糖皮质激素、PPI及营养支持治疗6 mo, 取得较好临床疗效. 但本病例随访上需关注的问题如下: (1)病情变化情况, 尤其是激素减量和停用之后病情转归及采用何种药物序贯治疗问题; (2)糖皮质激素使用后的副作用监测情况; (3)胃肠道息肉癌变的可能; (4)并发其他疾病的治疗. |
