| 核心内容提要 |
无痛胃肠镜检查是一种消化道内镜检查, 可提供直观、高清的图像为消化科疾病诊疗提供依据, 麻醉技术的应用可有效消除患者检查过程中的恐惧与不适感, 但是老年患者常伴随器官功能减退、基础疾病多、药代动力学改变等特点, 对麻醉药物的敏感性增高、代谢减慢, 麻醉相关风险显著增加. 全身麻醉虽能满足无痛检查需求, 但易造成呼吸抑制、血压波动大、苏醒延迟、术后认知功能改变等. 右美托咪定是临床常用的麻醉药物, 能够作用于中枢神经系统起到镇静镇痛的效果, 同时具有抗焦虑、抑制交感神经兴奋且对呼吸影响轻微等特点. 本研究发现, 右美托咪定辅助全身麻醉应用于老年无痛胃肠镜检查的镇静镇痛效果较好, 有助于维持患者血流动力学的稳定, 改善苏醒质量, 减轻应激反应, 对认知功能的影响较小, 且安全性较高. |
