食管癌与下咽癌在共同危险暴露和区域癌化背景下具有显著伴行关系. 临床处理的关键不是单独治疗某一处肿瘤, 而是在初诊、治疗前评估和随访中建立“上消化呼吸道整体筛查-分期整合-功能保留优先-复杂病例多学科决策”的路径. 本文提出以病灶分期组合、器官功能和治疗耐受性为核心的治疗框架, 重点讨论早期双原发、局部进展同步病变、不可耐受根治治疗患者、既往放疗后食管癌及术后重建等难点. |
