肝门部胆管癌转化治疗以GC方案联合免疫(如durvalumab)、立体定向体放射治疗局部控瘤及HER2靶向治疗最具前景, 但疗效异质性、手术时机与患者意愿构成主要挑战. 亟需随机对照试验验证最佳方案, 建立统一可切除评估体系, 并探索程序性死亡配体-1、循环肿瘤DNA等生物标志物, 推动个体化精准转化. |
