长期使用抗血小板药物可增加消化道溃疡大出血风险, 威胁患者生命安全. 介入栓塞术选用明胶海绵栓塞, 能有效避免误栓、错栓等现象, 快速止血, 提高止血成功率. 介入栓塞术和抗血小板药物联合应用可提高止血有效率, 减少并发症。年龄、抗血小板药物使用时间、溃疡史、Hp阳性、休克、输血量、中性粒细胞绝对值、尿素氮、Forrest分级Ⅰa是抗血小板药物致老年消化道溃疡大出血预后危险因素, 可能与溃疡累及较粗血管, 出血量大, 血流动力学不稳定有关, 建议临床做好上述指标监测及干预, 以降低再出血发生率, 改善预后. |
